To find the root cause - 5 whys Technique

5 Whys Technique - This technique was developed at Toyota. Using this approach, one can bypass the assumptions and logic traps and climb up from the effect to cause as explained by its wiki article: 5 Whys

This technique demonstrates that going through this iterative process of seeking the cause backwards would bring the seeker to a point where the root cause shows up as some broken process.
